Online Request Forms
The following forms can be completed online to fulfill a variety of requests. When you are ready to complete your request, select the "Complete Your Online Request" button above.
Alumni Library Card Request
All Baylor alumni may apply for check-out privileges. Applicants must present a valid, government-issued ID card with their current address, and supply current contact information before an Alumni Library Card will be issued.
Baylor Libraries Online Experience Update Request
Use this form to report necessary edits or to request changes to the Baylor Libraries Online Experience. (includes websites, blogs, library guides, etc.)
Digital Materials Request
Use this form to request digital resources, including streaming video and other media. Depending on your need, we may secure an external digital resource for you or digitize materials we have on-hand or which you provide.
Electronic Resource Issue Report
Use this form to report difficulty accessing a specific database, e-book, or e-journal.
Library Instruction Request
Use this form to request library instruction for your class or department.
Current employees of Baylor University may designate a personal representative to check out materials from Moody and Jones Libraries, on their behalf. You may also use this form to request an OsoFast Proxy Account that allows others to order ILL and OsoFast Delivery materials on your behalf.
School of Music Recording Request
The Library holds more than 9,000 audio recordings of School of Music performances, including faculty recitals, student recitals, and ensemble concerts. The archive is complete mainly from 1980 to approximately one year ago and includes a few recordings from earlier years.
The TexShare Card is a statewide library card that lets you borrow books and other physical materials from more than 500 libraries across the state. Click here for more information about TexShare.
